The Japan Foundation Website User Survey
The Japan Foundation is conducting a user survey in order to better
serve you with more useful, easier-to-understand content in our website.
We hope as many of the users as possible will reply to this survey.
The time required to respond to this survey is about three minutes.
The survey participants will have a chance to be one of 50 people to receive the prizes described below.

Please fill in the survey and mandatory information, and send the form.
Please note that your participation will be invalid if your form is
not complete.
[Notification of Winners]
Winners will be selected by the lottery and shipment of prizes serves
as notification.
Please note that we will not answer your query about the winner or
shipment of prizes.

Survey participants who wish to be included in the prize lottery
need to enter the following information:
Name/Postal Code/Address/Phone Number/E-mail Address
Your personal information will be used only for shipping the prize.
The survey data other than the above will be gained separately and
handled as no individual can be identified.
